Report a food or feed safety incident

Use this service to report a food or feed safety incident to the Incidents Branch of the Food Standards Agency. This form is for local authority representatives, other government departments and food business operators.

You will be asked to provide your contact details and information about the incident. This will include:

• The nature of the incident and its impact
• Completed and planned actions in response to the incident
• Details of the product(s) affected
• Each business involved with the production, supply and distribution of the product(s)

Please provide as much detail as possible, this will enable the Incidents Branch to provide advice as promptly as possible.

Information that is unavailable at the time of reporting can be sent in follow up to the form to foodincidents@food.gov.uk, please quote the reference number that you will receive on completion of the form in future correspondence. The Incidents Branch may contact you to request additional information.
